Parent Survey


Dear Parent,

Enclosed is a survey on the impact of alternate assessment for students with significant cognitive disabilities. The purpose of this survey is to understand the impact of alternate assessment from a parent perspective. Please consider completing this survey if you have at least one child who has participated in your state's alternate assessment sometime in the past three years. Your responses are anonymous; we will not be able to identify your answers. Your participation in this survey will take approximately 15 minutes. Please read the instructions for each section of the survey and fill in the bubble for each statement that best describes your experience with alternate assessment.

The benefit of completing the survey is that you have an opportunity to voice your opinion on alternate assessment. Teachers, administrators, and policy makers may find this study beneficial in improving the quality of alternate assessment that leads to enhanced students' outcomes. There are no foreseeable risks associated with this survey. Results of the survey may be submitted to a scholarly journal so that others may benefit from this new knowledge. Support for this research was funded in part by Grant No. H324C010040 of the U.S. Department of Education, Office of Special Education Programs, awarded to the University of North Carolina at Charlotte. This survey is being distributed to parents of children with disabilities across the country with the cooperation of the Parent Training and Information Centers and Community Centers.

Your decision to participate in this survey is completely voluntary. You may refuse to participate or stop your participation at any time. UNC Charlotte is eager to insure that all research participants are treated in a fair and respectful manner. If you have any concerns or questions about how you are being treated in this project, you should contact the Office of Research Services, Institutional Review Board for Research with Human Subjects (704) 687-3311

Your time and interest are appreciated. We are offering a prize drawing of a parent library of books worth $100 to be held in March 2004. The information you supply for the prize drawing will be stored in a separate file from the survey and can not be used to identify your surveyresponses.
